If you know anything about dental disease, then you know bacteria ranks high on the Usual Suspects list. Tooth decay gets its start from acid produced by bacteria; periodontal (gum) disease is often triggered by bacteria that infect the gums.
But the particular strains of bacteria that can cause dental disease are a small percentage of the 10,000-plus species inhabiting your mouth. The rest, numbering in the millions, are fairly benign—and some, as recent research is now showing, play a sizeable role in protecting your teeth and gums against other malicious bacteria, fungi and viruses.
Dr. Aaron Weinberg, a dental researcher at Case Western Reserve University, has been investigating these protective bacteria for many years. His research began with a scientific conundrum: although the mouth has one of the highest densities of bacterial populations, wounds in the mouth tend to heal quickly.
The answer, he believes, originates with human beta defensins (hBDs), substances produced by cells in the lining of the mouth that are natural antibiotics against disease. He has found that certain bacteria actually help stimulate their production.
This isn't just an interesting fact about the body's defenses and immune system. During his research, Dr. Weinberg was able to identify the agent within the bacteria that triggered hBD production. This has opened up a new line of research: The possibility that harnessing this agent might help assist in our treatment of infection by boosting the body's defensive capabilities.
For example, researchers have proposed including a form of the agent in toothpaste. Over time, this might stimulate hBD production and guard the mouth against the development of dental diseases like gum disease.
These possibilities all come from our increasing knowledge and understanding of the microscopic world around us, especially in our mouths. Bacteria are much more complex than we may have realized—not all are our enemies, and some are definitely our friends. Learning more may open up new ways to keep our teeth and gums healthy.
During this year's National Public Health Week in April, health issues like vaping and the opioid crisis are taking a back seat to what is front and center on everyone's mind: the novel coronavirus (COVID-19). This highly contagious viral infection is upending business as usual for most of the world in a way unlike anything we've experienced. Nothing is “normal” right now, including dental care.
As with other aspects of daily life, you can expect disruptions in dental care because of COVID-19, especially involving routine visits. If you do need to visit the dentist for treatment, you might be concerned about potentially exposing yourself or others to COVID-19. Like every business that interacts with the public and especially all healthcare providers, dental offices are implementing extra precautions during this time to protect both patients and staff against infection.
This isn't something new: The dental profession as a whole has strict protocols for preventing infection that have been in place for several years. Infection control is a top priority for dentists at all times, not just during outbreaks like COVID-19. Here are some of the things we do—and are expanding because of the novel coronavirus—to keep you safe during dental appointments.
Barrier protection. Dental providers routinely use disposable items like gloves, face masks or eyewear to prevent disease spread during procedures that involve close contact with patients. For extra precautions with COVID-19, we're adding more of this type of barrier protection.
Sterilization and waste disposal. Instruments and equipment that we use repeatedly are thoroughly sterilized to remove all microorganisms, including coronavirus, from their surfaces. For disposable items used during treatment, we keep these separate from common waste and dispose of them according to strict protocols for handling bio-medical waste.
Disinfection. Even though the main pathway for spreading COVID-19 is through respiratory droplets in the air, we're continually disinfecting office and treatment surfaces that the virus might potentially contaminate. In doing so, we're using substances recommended by the U.S. Centers for Disease Control (CDC). One of the most popular subjects in books, magazines and social media is food — the things we should or should not eat (or at least not too much). While losing weight is a popular focus, it's only one part of the whole — a balanced diet that supplies the nutrients we need to be healthy.
What you eat can also make a difference in your oral health. Here are 4 changes you should make to your dietary habits to cut down on the risk of dental disease.
Adopt a nutritionally sound diet plan. When we say diet, we're not talking about the latest weight-loss sensation — we mean a planned way of eating for life. For most people, that's a balanced diet of fresh fruits and vegetables, protein and dairy. Your teeth and gums have the best chance of remaining strong and healthy with a nutrient-rich diet.
Manage your sugar intake. Sugar and similar carbohydrates are a rich food source for bacteria that cause dental disease. It's important then that you keep your sugar consumption within limits: don't eat more than six teaspoons of processed sugar a day (or three for a child); avoid sugary snacks between meals; and try to satisfy your sweet tooth with the natural sugars found in fresh fruits and vegetables.
Cut back on acidic beverages. Sodas, juices, sports and energy drinks are all the rage. They're also high in acid, which at chronic levels can soften and erode tooth enamel. So, try to drink them only at meal times and avoid sipping on them over long periods. And, if you're hydrating yourself after moderate work or exercise, try nature's perfect hydrator — water.
Avoid eating before bedtime. A good portion of the acid in our mouths after we eat can be neutralized by saliva. As we sleep, though, our saliva flow slows down and doesn't have the same buffering power as it does during the day. So, try not to eat as least an hour before you turn in for the night, especially foods with added sugar.

Practice prevention now. Dental disease doesn’t come out of nowhere — it’s the product of a bacteria-rich environment and neglect. You can help eliminate that environment by removing plaque — a thin film of bacteria and food particles built up on tooth surfaces — with daily brushing and flossing. Twice-a-year dental cleanings remove plaque and calculus (hardened plaque deposits) you can’t reach with daily brushing. Reducing sugar (which bacteria feed on) in your diet and treating low saliva flow (which can increase decay-causing acid in the mouth) will round out your prevention practices.
Take care of emerging problems as soon as possible. Dental disease typically doesn’t go away by itself: more likely, it will get worse — and more costly — with time. Don’t wait to see us if you encounter tooth pain or bleeding, tender or swollen gums. In some cases, we can take temporary measures like resin-based fillings in decayed areas that can buy a little time while you prepare for the expense of a more permanent restoration.
Adopt a long-term care strategy. Our goal is for you to have as healthy a mouth as possible.Â To that end, we’ll work with you on strategy and payment plans that address your individual needs. A good strategy puts a priority on treating emergencies or advanced disease first, followed by treating less affected teeth as you’re able to afford it. We may also be able to address your tooth and gum problems with fewer but longer sessions that can help ease pressure on your costs.
Adopting solid hygiene and dietary habits now, visiting us at least twice a year and following a plan to treat problems as they emerge is your best approach for keeping dental care from making a huge impact on your wallet.

There’s been a growing awareness about the effects of gluten, a protein found in grains like wheat, rye and sometimes oats, on certain people. An estimated 1 in 133 Americans have Celiac Disease (CD), a gluten-related disorder that causes the body’s immune system to work against itself. And if you have CD, you could eventually face dental problems like enamel pitting and erosion.
When a person with CD consumes gluten, their immune system mistakenly identifies the protein as malicious and attacks it. The attack occurs in the membranes that line the digestive system, which in the process destroys cilia, tiny hair-like structures that aid in food absorption. This disrupts the body’s normal absorption of nutrients, which can lead to a number of systemic conditions including intestinal cancer.
Because of the lack of nutrients, your teeth’s enamel may develop defects. You may begin to see dull spots or pitting, or chalky grooves in its normally shiny surface: this is a sign you’ve lost surface enamel crystals (decalcification). You may also be more susceptible to outbreaks of aphthous ulcers (canker sores).
Because symptoms can be misdiagnosed or go unnoticed, it may be years before you know you have CD. You can, however, get a definitive diagnosis through a blood test for gluten antibodies, which is then confirmed with a biopsy of a tissue specimen from the intestine.
While there’s ongoing research for CD-related medication, there’s currently only one recognized treatment for it — remove gluten from your diet. This is much harder than it sounds, and requires knowing what you can and can’t eat, along with strict monitoring of food package labeling. Thankfully, the world is becoming better educated in this respect as more food manufacturers are clearly labeling products containing gluten and restaurants are providing gluten-free menu options.
Once you have dietary controls in place, your dental issues can be treated as any other person, with one exception: none of the products used in treatment like polishing paste or fluoride gels should contain gluten, and must be verified before using.
CD is a serious condition that could even become life-threatening. Knowing you or someone in your family has it will help you protect both your overall health and your teeth.
